Thank you for choosing Garrett Williamson's Out-of-School Time (OST) program! Out of School Time is for students in grades first (1st) through fifth (5th) when schools are not session (Bank holidays, In-services, etc).

You will need to complete and submit the following items to enroll. This form will need to be filled out yearly. If any changes of health, allergy, contact information, or address occur througout the year, please notify Garrett Williamson asap so we can have the most up to date records.

 

OST program fee, per child: $110/day (Paid via check, money order, procare or credit card; Please make all checks payable to Garrett Williamson.)

AGREEMENT AND WARNING: I understand and acknowledge that, except for limited circumstances listed below, an agritourism activity provider is not liable for any injury to or death of a participant resulting from an agritourism activity. I understand that I have accepted all risk of injury, death, property damage and other loss that may result from an agritourism activity. 1. Performs an act in a grossly negligent manner and causes injury or damages to a participant. 
2. Purposefully causes a participant’s injury. 
3. Acts or fails to act in a way that constitutes criminal conduct. 
4. Recklessly fails to warn or guard against a dangerous condition that causes injury or damages to a participant. A dangerous condition is a condition that creates an imminent and substantial risk of injury or damages to a participant.

Please note, the total below shows the amount due for all the program dates you have selected. If you've selected multiple dates, please note that payment is not due until one week before your child/ren's registered Out of School Time program.

I hereby grant permission for staff of the Garrett Williamson / Agriculture Education program to take whatever steps are necessary to obtain emergency medical care, if warranted. These steps may include, but are not limited to the following (and may not occur in this order):

1. Attempt to contact a parent or guardian.

2. Attempt to contact a child's physician.

3. Attempt to contact you through any of the emergency contacts listed.

4. If we cannot contact you or your child(ren)'s physician, we will do any or all of the following:

• A. Call 911
• B. Call the paramedies / an ambulance.
• C. Administer CPR and First Aid until medical help arrives.
• D. Have the child taken to an emergency hospital in the company of a staff member.

5. Any expenses incurred under Number 4 listed above, will be borne by the child's family.

6. Garrett Williamson / the Agriculture Education program will not be responsible for anything that may happen as the result of false information given at the time of enrollment.
